Notice of Permanent Closure of Medical Practice On December 23rd 2009, Dr. Knight permanently closed his medical practice in California and relocated to New Mexico. The San Diego office telephone number (619) 222-1114 is no longer answered, but instead goes directly to voice mail that is checked infrequently. I apologize for any inconvenience this may cause you, but I can assure you that this decision was not made lightly. It has been a pleasure and a privilege caring for you.

If you were
NOT a member of Sharp Community Medical Group and were last seen in my office after January 1, 2007: The urologists at
Hillcrest Urological Medical Group
assumed care of my non-HMO patients in 2009.
Your medical records were physically transferred to
Hillcrest Urological Medical Group
who became the official custodian of your medical records when my office closed
on December 23, 2009.

$35.00 processing fee payable by check or money order and a completed request
for Release of Medical Records before we can process your
request for the release of your medical record. Please print,
complete and sign a ‘Written
Instructions and Request for Release of Medical Records’ (see link below),
and mail the completed document and the $35.00 processing fee to:
