Notice of
Permanent Closure of Medical Practice
On December 23rd
2009, Dr. Knight permanently closed his medical practice in California and
relocated to New Mexico. The office telephone number is no longer be answered but instead goes
to voice mail that is checked infrequently. I apologize for
any inconvenience this may cause you, but I can assure you that this decision
was not made lightly. It has been a pleasure and a privilege caring for you.
Patients please
carefully read and follow the instructions below regarding obtaining Medical
Records and Continuity of Care:
Non-HMO Patients (patients
NOT members of Sharp Community Medical Group) and who were last seen AFTER January 1, 2007:
For your
convenience I have arranged for the excellent urologists at Hillcrest
Urological Medical Group to assume your urological care.
You medical records
have been physically transferred to Hillcrest
Urological Medical Group who are now the official custodians of your
medical records. I am confident that you will continue to receive some
of the best possible urological care available with these excellent doctors.

your medical records there is a $35.00 processing fee payable in advance by check or money order
that must
be received along with a completed request for Release of Medical Records
before we can process your request.

complete and sign a ‘Request for Release of Medical Records’ (see
Instructions and Request for Release of Medical Records)
and forward the completed document by along with your $35.00 payment to:
